Please read this informative paragraph in its entirety prior to filling out the Pre-Plan Online form below. It's important to know why and how this information is designed, to be used for both our benefit.

There are several important components to the online planner below.

1st being Primary Contact Information must be completely filled out with valid information, please check the accuracy of your data input.

This is considered the minimum information needed. If you leave any portion blank you will not be allowed to proceed or submit this form. Note after you fill in all the PRIMARY CONTACT INFORMATION you can skip to the last step (SERVICE REQUESTS) below and press "SUBMIT" to forward it to our preneed advisor. HOWEVER please keep in mind the other information we are requesting from you is all relative. Pun intended. We highly encourage you to fill it out. Below are the following reasons.

2nd A lot of the secondary information will be required. We suggest you fill it out to the best of your ability now for convenience here on this online form. This information will be transposed and used to fill out our Vital Statistics Form, which is mandatory for the State of Alaska Vital Records - ie used for your loved ones Death Certificate.

3rdly Ancillary information is incredibly important it's used by the funeral home to notify next-of-kin. You'd be surprised about the amount of folks that pass that we can't identify or notify their family because we don't have access to their contact information.

4th & lastly is adding & gathering information for the personalization of your advanced plan is not required at this time, however, if you know it & want to share it we can add it into your planning arrangements file.
